After having read this waiver, and in consideration of acceptance of my participation and Inspirit Jiu Jitsu furnishing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE Inspirit Brazilian Jiu-Jitsu, its employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in the Inspirit BJJ training, programs and or events.*
I do hereby give Inspirit Brazilian Jiu-Jitsu, its assigns, licensees, and legal representatives the irrevocable right to use my name, video, picture, portrait, or photograph in all forms and media and in all manners, including composite, for advertising, for publication or any other lawful purposes, and I waive any right to inspect or approve the finished product, including written copy, which may be created in connection therewith.*
